Oklahoma law requires all students entering 7th grade to have one Tdap vaccine (tetanus, diphtheria, and whooping cough) before they can attend school. Please bring your child’s immunization record showing the Tdap vaccine (with the doctor/clinic signature or stamp) when enrolling for 7th grade. If your child has already received the vaccine, they will not need another dose.
Parents may also request an immunization exemption form for medical, religious, or philosophical reasons (available online).
Need the vaccine? Free immunizations will be available through the Oklahoma Caring Van in front of the Catoosa Public Schools Dome on the following dates:
• April 14 (5:00-7:00 PM)
• May 1 (1:00-3:00 PM)
• July 7 (5:00-7:00 PM)
• July 20 (4:30-7:00 PM)
